Every family, every year needs to fill out this all-school permission form. It's quick and easy -- takes less than 5 minutes.
Does your child need a physical?
Wondering if your child needs to get a physical this summer? Here's a short list of who needs a physical on file for the upcoming school year:
- All new students starting this fall
- All kindergartners
- All high school students*
*Special Note: For returning TFS high school students who have a physical already on file with the school office from last year's sports seasons, your physical is good for 2 years according to MSHSAA. Mr. Waide will contact any students that need to submit a new physical for this school year.

We ask that you return the completed forms to school by July 31 so we can get our electronic records up-to-date before school starts. You can drop them off in person or mail to The Fulton School, 1100 White Road, Chesterfield, MO 63017. You can also scan and email the forms to [email protected]
Is your child up to date on vaccinations?
Be sure to review Missouri’s School Immunization Requirements for the coming school year:
If your child needs to get their immunizations updated over the summer, please send a copy of the doctor's immunization sheet to the school office so we can update our records. Does your child have a severe allergy?
If so, please print and have your physician fill out the Severe Allergy Form.
